Case Manager

We are looking for a Registered Nurse or Registered Dietitian to join our Case Management Team!  Case Management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs.  Our case managers are encouraged and supported to grow professionally and become Case Management Certified within 2 years of employment.

As a Case Manager, a typical day might include the following:

• Assessing patient needs to improve quality of life
• Communicating with patients by asking condition specific questions verses lecturing or telling patients to change
• Helping patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
• Collaborating with teammates with various medical backgrounds
• Promoting quality and cost-effective interventions and outcomes
• Going home for the day at 5:00pm.  Because spending time with family and friends is important.

This position might be for you if:

• You are looking for a Monday-Friday position with a set schedule and no overnights.

Administrative Assistant/Health Screener

We are looking to add an Administrative Assistant/Health Screener to our team.

In this position you will:

  • Provide administrative support
  • Answer multi-line phone per protocol and route to the appropriate department or staff member
  • Provide prompt response to phone inquiries
  • Monitor incoming faxes and distribute accordingly
  • Prepare weekly staff meeting agenda and record minutes
  • Data entry confidential information
  • Order, receive, organize and maintain office supplies
  • Prepare outgoing mail, including mass mailings
  • Upkeep company conference room and break rooms
  • Attend Wellness events and perform finger-stick blood screenings for glucose and cholesterol, manual blood pressure checks, BMI, height/weight measurements, waist circumference measurements and one-on-one education regarding results.
